All Minors Programs, including Exempt Programs, must be registered with the Office of Risk Management (ORM) at least four weeks prior to the start of the subject program. Failure to timely register any Minors Program will result in immediate cancellation of the event. To register the event, the Program Coordinator is to submit the completed form below.
