PILOT'S INFORMATION
(Must attach a copy of the Pilot's driver's licentse AND a copy of the Pilot's FAA 107 license (If FAA 107 is over 2 years old, you must also include a copy of their completion of the FAA recurrence)
DRONE INFORMATION
(Must attach a copy of the Drone FAA registration certificate)
FLIGHT INFORMATION:
(Must attach a copy of a map that shows the flight area (indicating max altitudes), take-off and landing locations, and RPIC Control Point)
The full details of a ticket, including any appropriate circumstances or supplementary information that may aid in resolving it.

You must include the following attachments: (If all required documents are not included this request will be DENIED.)
  1. Pilot's Driver License.
  2. Pilot's FAA 107 license (If FAA 107 is over 2 yrs old, you must also include the Completion of the FAA recurrence.
  3. Drone FAA registration certificate.
  4. A map showing the flight area (including max altitudes), take-off and landing locations, and RPIC Control Point.
  5. Certificate of Insurance with a minimum $1,000,000 for each occurence and $2,000,000 general aggregate.
