What Is It?
This form can be utilized to request Certificates of Insurance. This form can be used for a non-affiliated individual or organization to request a Certificates of Insurance related to off-campus rentals, events, etc. Former students may also use this form to provide employers with a Certificate of Insurance evidencing University insurance coverage during the former student's time as a University student (also called "evidence of coverage").
